A leading health organization seeks a Band 5 Healthy Child Programme Nurse to support local families and children through integrated health services. The role emphasizes health promotion, effective healthcare delivery, and community engagement. Candidates should be registered nurses with a commitment to reducing health inequalities and enhancing children's health outcomes.
As a registered nurse, the post holder will work within an integrated neighbourhood team contributing to and supporting the delivery of the Healthy Child Programme 0-5 services.
Working within Healthy Together, the post holder will contribute to improving access, identifying health needs, improving outcomes and reducing health inequalities by providing a comprehensive neighbourhood based service to meet the needs of local children and their families.
Through a whole family approach, the focus will be on prevention of ill health, promotion of positive health and well-being and working collaboratively with the wider community.
As a member of the Healthy Together neighbourhood team the post holder will be responsible for taking an active role with the 0-5 service in the delivery of integrated health services. Training and the competency framework will support all areas of work.
