A leading community health organization is seeking a registered nurse to join their integrated neighbourhood team. The role focuses on delivering the Healthy Child Programme 0-5 services, improving health access, and addressing health inequalities for local children and families. The successful candidate will be key in promoting health, conducting assessments, and collaborating with the community to foster well-being.
As a registered nurse, the post holder will work within an integrated neighbourhood team contributing to and supporting the delivery of the Healthy Child Programme 0-5 services.
Working within Healthy Together, the post holder will contribute to improving access, identifying health needs, improving outcomes, and reducing health inequalities by providing a comprehensive neighbourhood-based service to meet the needs of local children and their families.
Through a whole family approach, the focus will be on prevention of ill health, promotion of positive health and well-being, and working collaboratively with the wider community.
As a member of the Healthy Together neighbourhood team, the post holder will be responsible for actively participating in the delivery of integrated health services within the 0-5 service. Training and the competency framework will support all areas of work.
