Job Title: Occupational Therapist – Discharge Planning

Working Hours: 37,5 hours per week

An exciting and new opportunity has arisen for an experienced individual to join our client as a Band 5 Occupational Therapist with experience in discharge planning. Your experience may be as a physiotherapist, occupational therapist, nurse or an experienced discharge co-ordinator.

What will your responsibilities be?

The Transfer of Care Hub aims to prevent delays in patient discharge by coordinating with ward teams, senior management, and external agencies. The candidate will be ward-based, providing discharge advice and information on available support. They will ensure timely discharges, challenge decisions, and escalate complex cases to senior management. The role involves leading discharge planning, collaborating with multidisciplinary teams, and agreeing on discharge plans that meet patients’ needs. The candidate will also promote safe discharge practices and act as a resource for expert advice.

Qualifications and Experiences:

  • HCPC registration
  • Chartered Psychologist status and appropriate post-graduate qualifications
  • Previous experience of conducting research and project working
