This is an exciting opportunity to join our excellent planned community nursing team based in Boldmere, covering the four council wards around the area.
We are looking for a highly motivated applicant interested in working within the community, providing care to patients in their homes. You will work with a multidisciplinary team to achieve the best outcomes, ensuring patients' independence and well-being.
Responsibilities include working as part of a planned team delivering care in patients' homes. You should be competent in clinical skills such as diabetes care, insulin injections, and end-of-life care for patients who prefer to die at home. A passion for wound care and advocating for patients is essential.
