Job Responsibilities
- The post holder will:
- Support and deliver care to patients who have been admitted to the Community Health Team caseload and will coordinate with the team to approach care delivery on a day-to-day basis.
- Assess, plan, and implement a care plan for individual patients.
- Improve clinical outcomes for patients by enabling them to function independently, increasing their ability to remain in their own home/community, and reducing the need for or preventing inappropriate hospital admissions.
- Support joint integrated working across the Community Health Team and other stakeholders.
- Work closely with the Team Senior Nurse/AHP to ensure care delivery is of high quality and meets required standards.
Additional Duties
- Assess patients and gain their trust.
- Plan and deliver patients' care.
- Take samples, pulses, temperatures, and blood pressures.
- Monitor and administer medication.
