Job Purpose:This is a permanent Haematology Band 5 CNS support nurse post. The role will be an opportunity to enhance the skills and knowledge of a trained nurse whilst supporting the CNS team with their day to day tasks. It could provide the successful candidate with experience to build towards becoming an expert practitioner in cancer care. The post will provide support, education and experiential learning for a nurse to develop the practical and theoretical skills andknowledge to be able to deliver high quality cancer nursing care for patients with a cancer diagnosis and their families.1. Under the direction of a Senior Cancer Nurse the successful candidate will co-ordinate and deliver effective quality nursing care by assessing the needs, planning, implementing and evaluating the care for a group of patients.2. They will develop clinical expertise in all aspects of the cancer nursing field, ensuring the highest possible standard of nursing care.3. They will work as part of a multidisciplinary team.

Preference will be given to internal Trust staff, as well as 'Priority' and 'At Risk' status including NHS at Risk staff throughout Devon who is able to display recent relevant experience as dictated by the Person Specification.

Main duties of the job

The post holder will gain experience working in Cancer services . They will work within the malignanthaematology CNS services . Experience will be gained by working across in-patient and out-patient areas.

The successful individual will have the opportunity to gain competencies and confidence in the assessment, management, treatment andpsychological support of cancer patients and families. The skills and experience acquired will incorporate chemotherapy knowledge, symptom management, radiotherapy treatments, advancedcommunication, end of life experience and skills in audit.

Job responsibilities
  • Clinical Providing and delivering a high standard of nursing care by initiating and co-ordinating theassessment, planning, implementing and evaluating of individual patients needs based onevidence. This includes all aspects of care from admission to the planning and organisation ofdischarge arrangements with voluntary and statutory agencies to ensure continuity of carefrom the hospital to the community or continuing care setting. Also willing to provideemergency ward cover if required. Reporting and documenting all relevant nursing/patient needs, communicating changes withother health care professionals in the multi-disciplinary team with ongoing responsibility forpatient care as they occur. Providing and delivering health information, education and promotion within the clinical areaand advise patient, family, and others in a manner which maintains an individuals privacy anddignity. Engage and interact with carers and family, encouraging them to be actively involved inpatients care planning as outlined in ward / unit philosophy. Supports patients, their families and carers during the delivery of bad news, caring for theterminally ill and in the event of death. Demonstrating and undertaking evidence-based practice playing an active part in theintroduction of this evidence to improve patient centred care. Implementing and sharing good infection control practice. Ensure adequate precautions takento minimise the risk of cross infection in relation to the safe disposal of bodily fluids (includingblood). Undertake Trust updates to minimise risks to patients & staff.With appropriate training and assessment Management of adverse effects of Systemic therapy including neutropenia, anaemia andthrombocytopenia. Recognise, react to and manage appropriately emergencies related to this speciality. Assist with the organisation of urgent admissions and transfers to the unit in accordance withTrust policies.Managerial Initiating the reporting of an untoward incident such as complaints, clinical emergencies, andinjury or drug administration errors and taking immediate actions as appropriate. Contributing towards clinical projects to set and maintain standards of practice in conjunctionwith other multidisciplinary team members, participating in audits and quality monitoring. Managing the Clinical risk relating to individuals or a group of patients care, reporting issuesto appropriate senior staff. Demonstrating an awareness of and compliance with Health and Safety regulations/ Proceduresthat apply to staff, patients and visitors within the trust premises, reporting any deficiencies inequipment, furnishings or matters of hygiene.Educational Assess educational and information needs of patients and families/carers and devise plans toensure their needs are met. Keep educationally and professionally updated and meet the criteria of the NMC revalidation.Attend relevant courses to attain standards of specialist nursing. Participate in clinical audit programme or research projects. Supervise students and non-qualified staffProfessional Maintaining, sharing, contributing and continually developing an in depth specialist knowledgeof nursing practice relevant to the individual clinical area. Assisting other clinical areas within the Trust if the clinical situation or staffing levels demand. Supporting team members in delivering their objectives through offering advice, guidance andsupport as appropriate. Maintaining personal and professional development to meet the changing demands of the job,in line with professional registration requirements. Maintain an up to date record of professional development. Maintain and provide evidence of continued registration with NMC. Ensure up to date knowledge and competence in use of clinical equipment regularly used andmaintain a personal record of training.Communication and working relationships Maintain good relationships between all members of staff, thus developing professionalismthrough the area of work. Ensure good communication both verbally and via formal documentation with all members ofthe nursing and multidisciplinary team. Completes patient documentation fully, ensuring that all entries are accurate and legible andthat information systems are maintained. Refers to and works in partnership with the patient and family and other health careprofessionals in care planning. Able to communicate effectively in demanding and challenging situations, e.g. end of lifeissues. Be aware and able to locate all policies relating to good working practice within the clinical area. Ensure carers/family and visitors are aware of policies, standards/guidelines relevant to thepatients care or clinical area. Attending meetings as appropriate.Communicating/liaising with the multi professional teams including Ward sisters/Charge Nurses and all ward based nursing staff. Modern Matron. Educational staff.
