A leading NHS Trust is seeking a Band 4 Key Worker for its Community Mental Health Team in Wiltshire. The full-time role involves supporting individuals with mental health needs, contributing to their recovery goals, and promoting engagement with services. Ideal candidates will have experience in mental health support and the ability to work collaboratively within a team.
A great opportunity has arisen for a Band 4 Key Worker to join the North and East Wiltshire Community Mental Health Team. This is a full time position working Monday to Friday 9-5pm.
You would be joining a friendly, sociable, cohesive, and pro-active team and you would be well supported by those you work with.
The role is focussed on supporting people with severe and enduring mental health needs, and their carers, friends, relatives and their supporters in the community, enabling them to receive treatment in line with personal recovery goals, and facilitating engagement with mainstream services.
The post holder will contribute to the ongoing assessment, planning delivery and review of activities and interventions against identified health, social care and well being needs, acting as key worker for an allocated group of service users. The role will also require undertaking and delivering defined activities and interventions for a wider group of identified service users, in accordance with the agreed personal recovery plan. This maybe either on a one to one basis, or as part of a group activity.
