Job description

We have an exciting opportunity for a Discharge Co-Ordinator to join our friendly team on Sky A (Medical Short Stay Unit). We are looking to appoint an enthusiastic candidate with a friendly and helpful approach, and the confidence to effectively communicate with staff, patients, relatives, and external care providers (for example, care homes).

The post holder will work closely with the multidisciplinary team to plan, manage, and ensure safe discharges of patients who no longer meet the criteria to reside in an acute hospital setting. Responsibilities include completing daily board rounds with the multidisciplinary team and advocating for patients' discharge needs.

The role involves completing referral paperwork alongside ward clinicians to ensure safe discharges, including D2A and Fast Tracks. The post holder must manage a caseload of patients, prioritize based on patient needs, and provide accurate discharge plans.

Additional duties include working independently to facilitate timely discharges, liaising with inter-disciplinary team members regarding discharge planning, monitoring predicted discharge dates, and highlighting potential delays or concerns to ward staff and managers.

The role also requires liaising with other agencies, patients, families, and carers to ensure effective communication and coordination for discharges. The post holder will consult with the Junior Sister / Charge Nurse on complex discharge issues, attend daily board rounds, and coordinate with nursing, medical, therapy staff, and external agencies to ensure all necessary equipment and documentation are in place prior to discharge. Where appropriate, liaising with care homes and escorting patients for safe and effective discharges is also part of the role.
