Qualifications

  • NVQ 3 in Health & Social care or level 3 or equivalent required. Foundation degree in relevant subject if not completed.
  • NHS community rehabilitation or nursing experience: experience of working as a band 3 rehabilitation/therapy assistant/healthcare assistant/support worker.
  • Understands health and social needs of the local community.
  • Confidentiality and information governance awareness.
  • Ability to work independently with home and clinic visits.

Responsibilities

  • Work collaboratively within the Integrated Neighbourhood Team and wider MDT to deliver high-quality, patient-focused interventions.
  • Contribute to assessment, planning, implementation, and review of individualised care and rehabilitation plans.
  • Undertake delegated assessments and treatments within scope of practice.
  • Deliver safe, effective, and compassionate care to a diverse patient population.
  • Demonstrate ability to work independently while maintaining teamwork and communication.
  • Support team functioning including administration, point of contact for patients/families, and equipment maintenance.

Education

Job Introduction

We are seeking an enthusiastic Band 4 Nursing Associate/Assistant Practitioner to join our dedicated team, delivering high-quality, person-centred care that supports patients to achieve the best possible outcomes.

This advert is open to those looking for either Nursing Associate or Assistant Practitioner roles. Both roles are essential in delivering integrated care across the population. Nursing Associates are equipped with the knowledge, understanding, skills, attitudes and behaviours relevant to employment; and will act in accordance with the NMC code of professional standards of practice and behaviours for Nursing Associates. Nursing Associates work primarily within the nursing service.

Assistant Practitioners are equipment with the knowledge, understanding, skills, attitudes and behaviours relevant to employment. They will provide high standard of care and/or rehabilitation to a range of patients. Assistant Practitioners can work specifically within therapy, nursing or a combination of both services with the appropriate training, support and guidance.

This is a part-time role working 33.5 hours per week.

  • Work collaboratively within the Integrated Neighbourhood Team and wider MDT to deliver high-quality, patient-focused interventions.
  • Contribute to the assessment, planning, implementation, and review of individualised care and rehabilitation plans.
  • Undertake delegated assessments and treatments within their scope of practice and competence.
  • Deliver and promote safe, effective, and compassionate care to a diverse patient population with varying health and rehabilitation needs.
  • Demonstrate the ability to work independently while maintaining strong teamwork and communication skills.
  • Show initiative in supporting the overall functioning of the team, including administrative tasks, acting as a point of contact for patients, families, and partner organisations, and assisting with the maintenance and organisation of equipment and resources.

The ideal candidate will have -
  • NVQ 3 in Health & Social care or level 3 or equivalent. Requirement to commence Foundation degree in relevant subject if not already completed
  • NHS community rehabilitation or nursing experience: Experience of working as a band 3 rehabilitation/therapy assistant/healthcare assistant/support worker
  • Understands the health and social needs of the local community
  • Understands the relevant issues of working in the community including lone working
  • Understands issues of confidentiality and information governance
  • Acts on own initiative and problem solves
  • Works independently and carries out directed tasks with patients in their own home and clinic settings
  • Sets and maintains personal and professional boundaries. Independently completes required tasks and manages workload Excellent written and verbal communication skills
  • Excellent time management, flexible approach to workload
  • Excellent time management the required timescale
  • Qualification relevant to knowledge of healthcare, nursing, rehabilitation or therapy
  • Facilitating rehabilitation groups with Allied Health Professionals
  • Community nursing as a healthcare assistant experience

  • Flexible approach to workload
  • Able to use and take initiative to seek additional work to assist the team as a whole (e.g., team administration tasks, being the first point of contact for patients, families and other health professionals, assisting in the maintenance of service equipment)
