Job description

ROLE: Band 4 Health Care Assistant (Ref; R20) – Female Only Role

TEAM: Redbridge Mental Health and Wellbeing Team South (GWN2)

WORKING HOURS: Monday to Friday, 9am to 5pm

DURATION: 3 Months

RESPONSIBILITIES:
  1. Support peers and team leader, working effectively both as part of a team and independently.
  2. Ensure individuals with Severe & Enduring Mental Health receive annual health checks.
  3. Refer to specialist services when anomalies are identified.
  4. Collaborate with mental health professionals to promote recovery principles and maintain physical health.
  5. Support collaborative working by liaising with other health and social care professionals, especially Primary Care Network staff.
  6. Complete health screens for service users with severe mental illness, input and transfer health screen records from NELFT RIO to GP database (EMIS).
  7. Handle all administrative duties related to the clinic, including booking appointments, sending reminders, and follow-ups.
