The Blaenau Gwent Community Resource Team are seeking an exceptional Assistant Practitioner to join our dynamic integrated team. The Community Resource Team is a collaborative service between Aneurin Bevan University Health Board and Blaenau Gwent Social Services, dedicated to providing individuals with the right intervention at the right time, from the right professional(s). As an Assistant Practitioner, you will have the chance to make a real difference in the lives of adults in need of support to remain in their own homes. You will be an integral part of the wider multi-disciplinary team comprising of Physiotherapists, Occupational Therapists, Health and Well Being Support Workers, Social Workers, Medics and Nursing staff. You will work alongside other services such as the Rapid Response Team, Community Hospitals, Acute Hospitals, Home Care and Care Agencies.
The Community Resources Team's function is to work with people to enable them to be supported in their own home wherever possible, by preventing hospital admissions, facilitating the timely discharge of service users from acute and community hospitals and assessing for potential long term care and support needs.
The post holder must be able to identify service users who require preventative or therapeutic interventions and also demonstrate independent caseload management within a broad framework which is laid down and supervised by Health and Care Professions Council registered professionals. You will be expected to carry out treatment programmes, using good problem solving, time management and assessment skills, to meet the service users individual goals and outcomes, adapting where change is needed, to respect service users privacy, dignity, choice and diversity.
We aim to deliver high quality health and social care services for adults in Blaenau Gwent and the role in an integral part of the Community Resource Team. The service is provided 7 days per week, between the hours of 7:30 am - 8pm, 365 days per year. Staff are required to work their hours across 7 days per week so you will be expected to work weekends and bank holidays on a rota basis.
The ability to speak Welsh is desirable for this post, English and/or Welsh speakers are equally welcome to apply. A current full driving license is essential, the post holder must have access to a car during contracted hours.
