B6 Specialist Learning Disabilities Community Nurse - CHC

Would you like to work with a highly skilled and experienced multidisciplinary team?

Due to an internal promotion, an exciting opportunity has arisen for a registered nurse with a focus on learning disabilities (RNLD) or a Registered Adult or Mental Health Nurse with experience in working with people with learning disabilities, to develop their skills by joining our county-wide Learning Disability Service.

We seek a nurse eager to work with individuals with complex health needs and/or challenging behaviour/mental health issues. The role primarily involves assessment and case management of individuals funded under NHS Continuing Health Care (CHC).

We offer regular supervision and opportunities for professional development, with close support from an Advanced Nurse Specialist within the team.

The role is based at one of our Learning Disability Wiltshire locations in Chippenham, Warminster, Marlborough, or Salisbury, with a hybrid working model expected to involve 60% of time from a base.

A valid UK driving licence and access to a vehicle are essential due to the nature of the role.

Main duties of the job
  • Promote health and wellbeing of people with learning disabilities through clinical and educational interventions.
  • Provide specialist advice on learning disability health issues.
  • Work within a multidisciplinary team with other health professionals, local authorities, care providers, and families.
  • Be professionally accountable for a designated complex community nursing caseload, including providing advice or care management for clients assessed as eligible for CHC funding or under Section 117 of the Mental Health Act.
  • Support and contribute to the development of the learning disability service in line with government policies like Valuing People Now 2010.
