The Community Mental Health Recovery Service provides assessments for evidence-based, time-limited treatments and psychological interventions for adults with severe and enduring mental health issues.
The service focuses on promoting recovery and independence, developing self-management strategies to help individuals achieve their potential in the community.
We are a multidisciplinary team offering advice and support to GPs, working with individuals and their carers, and referring or signposting to various statutory, independent, and voluntary organizations. We aim to work closely with individuals, supporting their recovery journey, and their families, with consent.
Candidates must have a full and valid driving license and access to a vehicle for work purposes due to community involvement.
Main duties include coordinating and monitoring person-centred healthcare, assessing and planning care, respecting individual customs and beliefs, maintaining professional relationships, recording contacts accurately, identifying concerns, managing risks, recognizing potential crises, and utilizing clinical supervision for performance development.
