Job Overview

An excellent opportunity has arisen to be part of our transformational multi-agency HOMEFirst Alliance. This initiative uses multi-agency collaboration to provide recovery-focused community support for individuals with complex mental health difficulties, often leading to hospital admissions. The role is fixed-term for 12 months.

We welcome applicants with professional registration in Occupational Therapy, Social Work, or Nursing. A full driving license and access to a vehicle are required due to community-based work.

Key Responsibilities
  1. Provide high-quality, evidence-based assessments and interventions (group and individual) in line with the HOMEFirst model, with supervision from a Lead Occupational Therapist.
  2. Communicate effectively with service users, carers, multidisciplinary teams, and outside agencies.
  3. Contribute to team functioning and participate in service development and audits.
Additional Information

The post is based at Trust HQ in Leatherhead, working across Surrey with virtual elements included. The role involves community work, requiring a valid driver’s license and access to a vehicle.
