Job summary

South Tyneside and Sunderland NHS Foundation Trust is seeking a Nurse Practitioner to join the Sunderland Community Frailty Team. The role focuses on autonomous assessment and coordination of care for older adults with frailty in community settings, including care homes and MDT involvement.

You will conduct CGAs, develop care plans, and lead complex assessments with support for investigations and prescribing as needed. The post offers opportunities for innovation and professional development.

Qualifications

Registered Nurse with NMC registration.
Nursing or related degree.
Advanced clinical assessment skills accreditation or willingness to work towards.
Non Medical Prescriber or willingness to complete extended prescribing training.
Car driver with ability to travel across sites.
Leadership and management course.

Responsibilities

Coordinate the delivery and development of the specialist service through evidence-based practice in line with national standards for patient safety.
Plan and deliver safe, effective, compassionate patient care for dedicated care pathways as an autonomous practitioner.
Continuously improve clinical care standards within the governance framework.
Exercise accountability as set out in the NMC Code and ensure effective internal and external communication regarding patient care and resources.
Provide education and training to clinical staff within the Trust; offer specialist advice and support to patients and staff.
