Job summary

NHS Jobs is seeking a Locum Out of Hours ACP/Prescribing Pharmacist/Paramedic to provide autonomous clinical assessment and decision-making across adult inpatients during evenings, weekends and bank holidays.

You will perform triage, non-urgent clinical reviews and contribute to care pathways across rehabilitation, frailty, sub-acute and palliative settings, coordinating with nurses, GPs and emergency services to ensure timely, safe patient care.

Qualifications

• Qualified and registered with the relevant professional body.
• Experience in clinical triage and patient assessment.
• Ability to work autonomously within scope of practice.
• Willingness to work evenings, weekends and bank holidays.

Responsibilities

• Provide autonomous clinical assessment and decision-making.
• Undertake clinical triage and responsive care including remote reviews.
• Prescribing support and medicines optimisation where qualified.
• Support VTE assessment and safe escalation in admissions.
• Contribute to end-of-life care and escalation planning.
• Maintain accurate clinical documentation in electronic systems.
• Engage in governance, safety and quality improvement.
• Collaborate with multidisciplinary teams for patient care.
