Job summary

NHS Jobs in Stockport is seeking a General Practitioner to provide comprehensive medical care within the practice rota. You will conduct surgery and telephone consultations, visit patients at home, and manage repeat prescriptions, ensuring timely documentation and communication.

You will make professional autonomous decisions, assess undifferentiated health needs, develop care plans, and contribute to quality improvement through audits and team collaboration within the Practice/PCN framework.

Responsibilities

• Make himself/herself available to undertake a variety of duties including surgery consultations, telephone consultations and queries, visiting patients at home, checking and signing/authorising repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion.
• Make professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation.
• Assess health care needs of patients with undifferentiated and undiagnosed problems.
• Screen patients for disease risk factors and early signs of illness.
• Develop care plans for health in consultation with patients and in line with current practice disease management protocols.
• Provide counselling and health education.
• Refer to other care providers as appropriate.
• Record clear and contemporaneous consultation notes to agreed standards.
• Compile and issue computer-generated acute and repeat prescriptions.
• Prescribe in accordance with the practice prescribing formulary (or generically) when clinically appropriate.
• Be available to visit patients during surgery time in an emergency if clinically indicated.
• Participate in the extended hours enhanced service.
• Undertake fair share of home visiting sessions per week as allocated.
• Gain experience with audit and be able to lead audit programmes.
