Job summary

Mersey Care NHS Foundation Trust is recruiting a Band 4 Community Registered Nursing Associate for the Sefton area. You will join a supportive community district nursing team delivering care in patients' homes and community settings.

The role involves autonomous working, a 37.5 hour week, and shifts between 08:00 and 20:00 across days and weekends, with flexible working considered. You will embody compassionate, patient-centred care and collaborate with colleagues to maintain high standards.

Responsibilities

• Assist in delivering high standards of nursing care to patients in the community following prescribed care plans.
• Work autonomously and as part of a community health team to support patient care in homes and community settings.
• Provide competent and skilled care and advice to patients and carers, ensuring care is patient-centred.
• Be flexible, approachable and motivated, able to use own initiative and work across days, weekends and bank holidays.
• Contribute to clinical leadership within the community team and support delivery of policies and procedures.
