Job summary

Hampshire and Isle of Wight Healthcare NHS Foundation Trust is seeking a Mental Health Practitioner for the South Wight community. You will work within a multidisciplinary team, managing a caseload of individuals with complex needs and delivering evidence-based interventions in the community.

Qualifications

• NMC registered RN [MH] or qualified OT or Social Worker with current registration.
• Evidence of on-going professional development.
• Post registration training in evidence based psychological treatments.
• Care Co-ordinator competencies.
• Training in risk assessment and management.
• Ability to work across disciplines.
• Experience in assessment and management of severe mental health cases.
• Audit experience.
• Innovative and creative with a willingness to try new ideas.
• Nurse Mentorship qualification or equivalent.

Responsibilities

• Provide high quality care and treatment with other professionals to meet needs.
• Work with services to deliver seamless, flexible community care.
• Provide care, interventions and health promotion to people with severe mental health problems in the community.
• Undertake supervisory role with junior multidisciplinary team members and advise on clinical issues.
• Work as an autonomous practitioner and engage in training and teaching within the Trust and community.

Education

RN [MH] with live NMC registration or OT or Social worker with current professional registration
