Job summary

BrisDoc in the Severnside area seeks a Clinical Practitioner to work across Severnside Integrated Urgent Care (IUC) services including The Burren, Out of Hours, CAS, CMDU and ACE. The role involves autonomous prescribing, managing care for patients discharged into community placements, and contributing to high-quality primary medical care within the clinician’s scope.

Supported by the Nursing and AHP Leadership Team and Primary Care GPs, you will work within a multidisciplinary team to provide

Responsibilities

• Assess, diagnose, plan, implement and evaluate treatment/interventions for undifferentiated or complex conditions.
• Clinically examine needs and plan care from physiological and psychological perspectives.
• Undertake home visits as required to support patients in the community.
• Compose robust safety netting and risk management for service users.
• Make effective referrals to system partners to ensure holistic care.
• Participate in MDT discussions to drive best practice and patient outcomes.
• Prescribers: prescribe and review medications within evidence-based practice and protocols.
• CMDU: participate in antivirals service as applicable.
• Communicate clearly to ensure patients are fully informed and consent to treatment.
