Job summary

University Hospitals Dorset NHS Foundation Trust is seeking a Band 6 Anticoagulation Practitioner to join our anticoagulation service in Bournemouth. You will manage over 1,000 patients on anticoagulation therapy, delivering expert dosing, education and patient support across hospital and community settings.

The role requires post-registration NMC registration (Band 5), experience in anticoagulation, and strong IT skills to operate dosing software.

Qualifications

Post-registration NMC registration is required.
Experience in anticoagulation or cardiology/stroke settings is advantageous.
Excellent communication and organisational skills.
Ability to work independently within a multidisciplinary team.
Commitment to patient safety and high-quality care.

Responsibilities

Provide autonomous clinical dosing for anticoagulants in hospital and community settings.
Manage a caseload and adjust treatment per guidelines.
Dose Warfarin and DOACs using computerised software per guidelines.
Deliver structured education and specialist advice to patients and families.
Conduct telephone consultations and clinics for treatment review.
Maintain accurate records and ensure compliance with NHS guidelines.
Support training and a learning environment within the team.
Contribute to service development and audits.
