Job summary

Leicester Terrace Health Care Centre in Northampton is seeking an Advance Clinical Practitioner to join our multidisciplinary GP practice. The ACP will work autonomously across all ages, including paediatric presentations, and contribute to home visits for housebound patients.

Qualifications

• MSc in Advanced Clinical Practice (or equivalent Level 7 ACP qualification).
• Evidence of advanced clinical assessment and diagnostic skills.
• Registered with NMC / HCPC / GPhC.
• Clinical portfolio demonstrating competence across ACP domains.

Responsibilities

• Provide autonomous assessment, diagnosis, treatment and evaluation for patients across age groups.
• Undertake paediatric assessments and manage acute conditions safely.
• Conduct home visits as part of the practice visiting team; frailty, chronic conditions, urgent presentations.
• Prescribe medications safely and appropriately in line with scope and legislation.
• Support teaching, CPD and supervision within the ACP framework.
