Job summary

Tameside and Glossop Integrated Care NHS Foundation Trust is recruiting for an ATR Tracker to monitor and update the ATR PTL, track patient diagnostic tests, and maintain accurate records across Trust systems. The role supports timely patient care by liaising with clinicians and hospital departments.

The role emphasizes data integrity, coordination across pathways, and the training of new ATR Trackers, within a community-focused NHS trust that values inclusive recruitment and staff wellbeing.

Qualifications

• Experience tracking outpatient test results from request to outcome.
• Proven ability to maintain accurate patient data records in clinical systems.
• Familiarity with electronic patient notes such as Lorenzo and other Trust systems.

Responsibilities

• Track all outpatient test results from request to outcome after clinical review.
• Collect information to support monitoring of patients on clinical pathways.
• Ensure complete, accurate and timely data is recorded in the Patient Administration System and other Trust systems.
• Liaise with other departments to coordinate care.
• Communicate patient progress through a pathway to clinicians and managers.
• Intervene when patients risk breaching referral-to-treatment/ATR targets and raise to management.
• Create and maintain databases or spreadsheets as required.
• Assist in training of new ATR Trackers.
• Handle potentially distressing information and correspondence professionally.
• Perform other duties commensurate with the grade and in line with post requirements.

Responsible for monitoring and updating the ATR PTL (Awaiting Test Results Patient Treatment List), tracking patient diagnostic tests, maintaining accurate records, and liaising with clinicians and hospital departments to support timely patient care.
