Salary: £40.69 per hour

Work Schedule: Tuesday to Saturday (Hospital Discharge)

Southwark Council is seeking an energetic and passionate Deputy Team Manager to oversee Hospital Discharge Transfers of Care during weekends. This leadership role involves managing a team to ensure smooth patient discharges and effective care transitions from hospital to home or other settings.

Responsibilities:
  • Oversee daily operations from Tuesday to Saturday, ensuring efficient triage, case allocation, and assessments.
  • Provide guidance on complex discharges, safeguarding, and mental capacity assessments.
  • Lead the management of weekend hospital discharges at Kings and St Thomas Hospital, and coordinate referrals to extra care schemes.
  • Mentor and supervise social workers and assistant practitioners, fostering professional growth.
Candidate Requirements:
  • Strong knowledge of the Care Act, Mental Capacity Act, and Continuing Healthcare processes.
  • Experience leading multidisciplinary teams and safeguarding meetings.
  • Ability to lead confidently, think creatively, and make informed decisions.
  • Supervisory experience of 4-5 social workers preferred.

Additional Notes: The role requires presence at St Thomas Hospital and Kings College Hospital approximately 4 days per week, with limited scope for working from home.
