Overview

Farleigh Hospice is a high performing organisation providing specialist palliative and end of life care for people with life limiting illness in the Mid Essex region. Our team at Farleigh has grown and evolved to meet the changing needs of the community we serve and we are looking for highly motivated Associate Practitioner to join our Therapy team.

As an Associate Practitioner, you will work within a multi-disciplinary team (MDT) to support the delivery of high-quality palliative and end-of-life care. You will assist in holistic assessments, provide emotional and practical support, and contribute to care planning and rehabilitation services under the supervision of registered professionals.

Responsibilities
  • Work within an MDT to support the delivery of high-quality palliative and end-of-life care.
  • Assist in holistic assessments and contribute to care planning and rehabilitation services under the supervision of registered professionals.
  • Provide emotional and practical support and engage in sensitive conversations with patients and families.
  • Facilitate group sessions, promote non-pharmacological symptom management, and support patients in accessing appropriate health and social care services.
  • Be proactive in problem solving; work independently and with supervision as required.
  • Contribute to service development, quality improvement initiatives, and maintain strong relationships with external partners and stakeholders.
Qualifications
  • Completed the Assistant Practitioner Foundation Degree course with either an Occupational Therapy or Physiotherapy module.
Contract and terms
  • This is a fixed term contract and will be reviewed at the end of the term with regards to the needs of the service.
