Associate Practitioner

You will provide practical support to service users in performing day-to-day activities, guidance and education. You will be responsible for the running of physical health clinics/requested checks by consultants. You will also act as a mentor to students and junior staff, and be responsible for supervising unqualified staff. You will be expected to work collaboratively with key partner organisations, service users, carers, clinicians and other practitioners within the multi-disciplinary team in delivering services. In delivering your duties, you will be expected to display behaviors that are aligned with the Trust values and as required in accordance with the NHS Constitution at all times.

Job responsibilities

Core Clinical Function

Conduct physical health care checks of service users and act on the findings.

Suppport the 1st leven nursing staff in running depot clinic

Provide timely feedback and documentation in line with Trust policy.

Undertake risk assessments of individuals and their environments. As and when required

Facilitate low-level group work such as relaxation, health promotion, and sleep hygiene.

Support medication regimes in line with Trust policy.

Be an Infection Control Link person (to support 1st level nurse).

Support service users in daily living activities including shopping and budget management.
