Are you an Assistant Practitioner or Nursing Associate looking for a new challenge? Would you like the opportunity to work in Community Services within NLaG? We have an opening for enthusiastic Band 4 Assistant Practitioners or Nursing Associates to join the Planned District Nursing teams across North Lincolnshire, working alongside District Nurses, registered Community Nurses, Health Care Assistants, and Phlebotomists.

You must be flexible, caring, professional, and able to remain calm under pressure.

Community Band 4 Nurses are an essential part of the District Nursing team, providing quality nursing care to patients in the community.
  1. Perform unsupervised delegated nursing duties as defined by the caseload holder.
  2. Develop and maintain effective communication channels with other professionals and agencies.
  3. Work within your scope of practice according to the NMC Code (2018) and NMC Standards of Proficiency (2018).
  4. Provide holistic and person-centred care and support for patients.
  5. Participate in audits and service improvement initiatives.
  6. Supervise and support HCAs, Trainee Nursing Associates, and Pre-registration nurses.
  7. Develop skills further based on NMC guidelines and the evolving Nursing Associate role.
