Role Summary
We are seeking an innovative, enthusiastic, and motivated Band 6 Associate Community Matron for a permanent contract to join our Urgent Community Response Team, working flexibly across the Central and East Exeter locality. Our team is an integrated health and social care unit that facilitates crisis management in the community, prevents hospital admissions, and supports appropriate discharges, helping individuals remain as independent as possible at home. This is an exciting, fast-paced environment offering daily new challenges and learning opportunities. The Associate Community Matron plays a pivotal role within the Integrated Health & Social Care team, supporting Primary care, Community Nursing, and the Multidisciplinary Team. Using clinical assessment skills and expert knowledge, the role involves comprehensive assessments of patients with acute and long-term conditions, including frailty. The successful candidate must demonstrate flexibility, excellent time management, and strong communication and interpersonal skills. Ability to work autonomously, sometimes lone working, is essential, along with being a committed team member.
The role also involves supporting the professional development of junior staff, focusing on competency and skill set, and applying clinical leadership in caring for patients with complex needs. Collaboration with Managers, Senior Nurses, and Clinical Leads to develop the service according to patient needs and Trust-wide initiatives is also key.
