We are seeking a dedicated Associate Palliative Clinical Nurse Specialist to join our dynamic and supportive Community Palliative Care Team. In this rewarding role, you will work closely with a Clinical Nurse Specialist and the wider multidisciplinary team to deliver high-quality, person-centred care to patients with life-limiting conditions in their own homes or care settings.

Main duties of the job

The post-holder will be jointly responsible for the management of a defined caseload (patients/service-users) and will provide specialist nursing support, advice, education and/or care to that specific group. They will carry continuing shared responsibility for the assessment of needs, the development, implementation and evaluation of programmes of care and the setting of standards within the Palliative Care Team under the supervision of a Clinical Nurse Specialist

  • Manage a defined caseload of patients, providing specialist nursing care, advice, and education.
  • Assess, plan, implement, and evaluate individualised care plans in line with best practice and clinical protocols.
  • Deliver holistic support - clinical, psychological, and social - to patients and their families.
  • Promote self-care and independence, aiming to reduce unnecessary hospital admissions.
  • Communicate complex and sensitive information with empathy and professionalism.
  • Contribute to service development, audits, and the implementation of national and local standards.
  • Support and educate other healthcare professionals and carers in palliative care best practices.
