Role Description

This part-time hybrid role is as an Associate Case Manager at Inclusive Case Management. The role involves assessing client needs, coordinating services, developing personalised care plans, and collaborating with clients, their families, and support networks. Some work from home is acceptable, with the role in Greater Manchester.

Qualifications and experience

• Qualification in Social work, Speech and language, nursing, occupational therapy - essential
• Assessment and Case Management Skills
• Coordination of Services and Care Planning experience
• Excellent communication and interpersonal skills
• Experience in developing personalised care plans
• Ability to work collaboratively with clients, families, and support networks
• Knowledge of rehabilitation services and resources
• Understanding of client-centred care principles
