Associate Advanced Nurse Practitioner - Community

Are you passionate about palliative and end of life care and looking fora new challenge? We are looking for someone who holds a high amount of empathyand compassion, and who can make a real difference not only to our patients butto their family and support network.

You will seek to provide a high standard of holistic, skilled,patient-centred, palliative care to all community patients, as part of Mountbatten Hampshiremultidisciplinary community services team, providing nursing care under the directclinical leadership of the Locality Lead/ Senior Nurse on Duty. You will needto be an enthusiastic and motivated nurse, keen to develop both your practiceand education, in line with the role requirements, and have been qualified forat least 2 years, having gained relevant experience.

You will need to hold a cleandriving licence and have the use of a car.

Full-time: 37.5 hours a week

Main duties of the job

The Associate Nurse Practitioner role will contribute to the provision of specialist palliative and end of life care in the Community, being based at Mountbatten Hampshire.

Under the supervision of the Locality Leads, they will work autonomously and as a member of the Community Palliative Care Team influencing the delivery of ongoing patient care.

Job responsibilities

To work across care settings providing advice, education and support to patients, their carers and other healthcare professionals, acting as an ambassador with external partners and other key stakeholders.

The role will promote preferred place of care, management of complex end of life care symptoms and managing staff, patient and carer expectations whilst meeting the integrated care needs of the patient.

KeyTasks:

Developand maintain communications with patients, carers, other professionals andvolunteers, developing specialist knowledge in palliative and end of life care.

Communicate information to distressed patients and/or bereaved family members/people in a sensitive manner.

Communicatewith patients in ways that empower them to make informed choices abouttheir health and social care. Act as advocate for individual patients andthe client group.

Towork collaboratively, building relationships with other members ofmultidisciplinary team and outside agencies to ensure individualised care isprovided in a timely and responsive way.

Provideadvice by telephone to patients, carers and other professionals, as requiredfrom the Community Hub.

Giveand receive accurate reports and changes in patients condition, linking withkey clinical staff where required, to facilitate comprehensive and safe patientcare.

Maintainaccurate and contemporaneous records on System 1 and in all areas of clinicalpractice.

Providea rapid response visit to a patients home when required, undertaking a fullclinical assessment of need, prioritisation and delivery of care.

Undertake joint clinical assessments with other professionals and liaise asappropriate regarding ongoing care.

Useof skilled decision making to determine appropriate prescribed medication toalleviate symptoms.

Demonstrateskills for assessing and interpreting any changes in a patient condition andreporting/ escalating to appropriate professional, where required.

Beaware of health and safety aspects of the service and adhere to lone workerpolicy.

Promotea safe environment for all patients, carers, visitors and staff in thepatients own home/inpatient unit as appropriate.

Effectively support the teamregarding Safeguarding and MCA, working closely with the leads withinMountbatten

Towork collaboratively, building relationships alongside colleagues, to offer thepatient and family the skills of a dedicated palliative care serviceencompassing symptom control, social, psychological and spiritual care.

Toparticipate in quality initiatives within the community.

Undertakeaudit and evaluation of the care provided in the community.

Engagein a personal education programme that builds personal knowledge, skills andcompetency in palliative and end of life care.

Qualifications

• First Level Registration with NMC and in possession of a relevant degree in Health/Nursing Studies
• Assessor/supervisor qualification and experience
• Full UK Driving Licence (clean) and use of vehicle with Business Insurance
