JOB DESCRIPTION

To ensure the wellbeing of individuals living in the community by supervising, maintaining and developing the standards of care and practice of care at home staff. To provide care to individuals in community in a flexible, efficient and effective manner which enables them to live as independent a life as possible.

The post holder will also play a key role in supporting and promoting a reablement approach across the service. The post holder will assist the Supervisor (Care at Home) in on-going developments of the service to support the change agenda associated with supporting individuals with complex needs being supported within the community.
