Job description
Overview

The post is part of the Camden Minding the Gap Partnership (MTG) and Islington Transitions Team (ITT), an initiative designed to improve the transition experience of young people (aged 17-25) from CAMHS and other youth services into adult services and to make services more accessible and user-friendly to young people. The post-holder will support the work of MTG and ITT by administering the referral meetings, liaising with services in relation to transition cases, and collecting data for Camden & Islington Commissioners and preparing data reports. In addition, the post holder will spend one day per week in one of the Young People Service Clinical Pathways (to be discussed and agreed with the Post Holder’s supervisor once they have started). The post holder may work across multiple sites within the Trust and time spent at each site will vary according to service need.

Main duties and responsibilities
  • To provide a full range of high-quality administrative support to the MTG/ITT Minding the Gap meetings, including, but not restricted to:
  • Capture data and prepare papers for quarterly reporting to commissioners for the service.
  • Assist in the design and implementation of research, audit and service development projects within the service as required.
  • Undertake data collection, analysis, the production of reports and summaries, using databases and statistical programmes as required, across several different electronic patient records as held by both CAMHS and Adult Services.
  • Accurately take and circulate minutes of meetings, type correspondence, reports, summaries and other documents, both clinical and general.
  • Manage the receipt of referrals and generate the agenda for the MTG/ITT Meeting.
  • Assemble information about cases in advance of the MTG/ITT meeting.
  • Support the action points from the MTG/ITT Meeting by liaising with partner agencies as required.
  • Interview clients to capture qualitative data around the experience of transition.
  • Financial responsibility – The post holder will hold responsibility as requisitioning officer for the Young People and Transitions Service; training will be provided.

In order to meet the needs of the Trust, you may be required from time to time to work at different locations to your normal place of work. This may mean that you are required to work at any location falling under Barnet, Camden, Enfield, Haringey or Islington. The Trust reserves the right to require staff to work at such other places or locations as it considers reasonable and necessary on a temporary or permanent basis.

Additional job roles & responsibilities
  • To assist in the psychological assessment, formulation and delivery of 1:1 or group-based psychological interventions for service users, under the supervision of a qualified professional psychologist.
  • To work with other staff in assessment, care provision and relevant multi-professional meetings.
  • Research, audit, and service evaluation.
  • To assist in the design and implementation of research, audit, quality improvement and service development projects within the services as required.
  • To undertake data collection, analysis, the production of reports and summaries, using databases and statistical programmes as required.
