As An Assistant Practitioner, You Will
Deliver proactive, short-term support to clients via telephone, virtual sessions, and face-to-face meetings.
Conduct risk assessments and collaboratively create safety plans.
Help clients navigate and access the most appropriate community-based services to meet their needs.
Support the service in gaining valuable insights into our client cohort to improve outcomes and service development.
This is an exciting opportunity to have a real and lasting impact—supporting clients at a critical stage in their mental health journey.
Key Responsibilities
Provide therapeutic 1:1 interventions and facilitate group activities.
Liaise with external agencies to promote holistic, partnership-based care.
Ensure accurate and meaningful documentation of service users' needs.
Identify and respond to physical and mental health needs.
Formulate and evaluate short-term support plans.
Participate in clinical supervision and contribute to team development.
Signpost and refer clients to appropriate local services across health, safeguarding, and social care systems.
About You
You'll be:
A compassionate and client-centred professional.
Skilled in risk assessment, safety planning, and engagement.
Experienced in working autonomously and as part of a dynamic team.
Passionate about ensuring timely, appropriate care for individuals facing mental health challenges.
Candidates need to hold a Foundation Degree in Health and Social Care/mental health or apprenticeship in H&SC/Mental Health L5.
