A community health services provider in Nottingham seeks a part-time Assistant Practitioner Speech and Language Therapist. The role involves providing rehabilitation to clients with swallowing and communication difficulties in various community settings. Candidates should be educated to a Foundation degree level or equivalent, with experience in health or social care settings. The position offers £26,530 to £29,114 a year pro-rata.
We are looking to recruit an enthusiastic andmotivated person to a part time permanent Assistant Practitioner post withinthe friendly, dynamic and supportive Adult Speech and Language Therapy team. Wework across Nottingham city and are currently based at The Meadows HealthCentre.
You will work as an integral member of the Adult Speechand Language therapy team, providing face-to-face rehabilitation to clientswith swallowing and communication difficulties in community settings, such astheir own home, care homes or clinic settings. A key aspect of the role is todeliver communication group therapy and training to others, including care homestaff, with supervision from qualified clinicians. You will alsoprovide support to the speech and language therapists by preparing therapyresources, such as communication picture aids, and collating medicalinformation from medical notes, patients and carers to support with the triageprocess. Having a specialised role in working with people with swallowingdisorders (dysphagia), such as carrying out screening assessments and reviewingswallow care-plans, you will be given the necessary support to complete ourCityCare competency framework for Assistant Practitioners, with team specificcompetencies in communication and swallowing disorders. You will also haveregular opportunities for continuing professional development through attendingour regular in-service training as well as relevant Clinical Excellence Networkmeetings
The Speech and Language Therapy team work across Nottingham,supporting a wide variety of adult clients in their own homes, communitysettings including clinics, care and nursing homes. We offer virtualconsultations and interventions. We work closely with other community healthand social care teams, and the voluntary sector. Our clients present with awide range of aetiologies including stroke, end of life, dementia, neurological,mental health and respiratory conditions.
Your responsibilities will include workingautonomously to meet individual patient needs. You will have the ability toplan ongoing clinical interventions e.g. home visits, organising appointments,clinics and assessments. You will also be an excellent communicator withpatients, carers and within the team. You will be quick to report any concernsyou have about patient care and regularly raise any questions or ideas relatingto the service with the rest of the team. You will have excellent rapport buildingskills with patients and their carers, as well as conflict resolution andnegotiation skills.
You will have knowledge and experience of patient care ina health and/or social care setting and will be educated to Foundation degree or equivalent, in a healthrelated discipline or have evidence of successfulcompletion of all Citycare Band 4 AP competencies.
The post consists of 30 hours per week
A driving licence/vehicle is essential.

Job Purpose

Key Responsibilities
Clinical
1.Toundertake individual treatment and interventions with patients under thesupervision of qualified staff.
To review clients and adapt individual and groupexercise programmes as appropriate
1.To referservice users to external agencies and specialists when their needs are beyondthe assistant practitioner role
2.Toaccept responsibility for a designated caseload, working independently andtaking an active role in promoting independence and meeting the needs of theservice user.
3.Topromote well-being, safe mobility, transfers and effective communication. To provide instruction, advice andintervention to service users and carers in line with the team guidelines.
4.Toassess, provide, fit and demonstrate use of a wide range of equipment topromote independence.
5.Toattend and contribute to team meetings and case discussions using effectivecommunication.
6.To leadgroups and carry out training packages under the supervision of qualifiedclinicians
7.Tomaintain comprehensive and accurate records in line with employers polices

8.Toprovide support to other Assistant Practitioners as appropriate.
9.To feedinto and support team service improvement initiatives
Education andDevelopment
10.Toensure knowledge, skills and competencies are kept up to date and complete allrelevant competencies within recognised time frames (as detailed in thecompetency guidelines).
11.To beinvolved in formal appraisal systems, supervision, and be active in identifyingaims and objectives for personal development in line with departmentalpolicy.
