Job summary

Mersey Care NHS Foundation Trust in Liverpool is seeking a Therapy Assistant Practitioner with a special interest and extensive experience in neuro rehabilitation to join our Community Neuro Rehab Team and support adults in their homes across the Liverpool footprint.

You will assess, plan and deliver personalised programmes, work with therapists to meet priorities, promote health, and travel across the organisation.

Responsibilities

• Assess, plan and implement individualised programmes of care within the skills and competencies expected of the role
• Work collaboratively within the team and with other providers to deliver local and national health priorities and objectives
• Deliver therapeutic interventions in line with national quality and standards and Trust policies.
• Actively support work colleagues and patients with compassion and courage acting as at their advocate and champion
• Be accountable for own clinical practice
• Work within the skills and competencies expected of the role
• Communicate effectively with patients and carers recognising the need for alternative methods of commination to overcome different levels of understanding, cultural background and preferred ways of communication
• Lead on health promotion
• Support clinical staff in managing long term conditions
• Participate in supervisions, development sessions, undertake all relevant mandatory training
• Participate in audits as required by the organisation
• Travel across the footprint of the organisation

We are looking for a Therapy Assistant Practitioner with a special interest and extensive experience in neuro rehabilitation to join our Liverpool Community Neuro Rehab Team.

Our team is a specialist integrated team of Occupational Therapists and Physiotherapists, working together to provide intervention for adults in the community setting covering the Liverpool footprint.
