Job summary

We have exciting opportunities for a Band 4 Assistant Practitioner to join the Community Children's Nursing Service. The role supports children with high complex physical needs, following safe delegation and under guidance from a Registered Nurse within the school environment.

Proficiency in Welsh is desirable; applications from English and/or Welsh speakers are equally welcome.

Main duties of the job

The Assistant Practitioner will work under the supervision of a registered nurse to provide high-quality, holistic, compassionate healthcare to children with complex needs within the school setting. The role involves managing own workload, undertaking delegated tasks, and delivering care in line with current evidence-based practices. The post holder will be responsible for some assessment tasks, implementing and modifying care plans, and reporting to the registered practitioner. Supervision, teaching, and assessment of other staff may also be part of the role.

Qualifications and knowledge
  • Certificate in Health Care Level 4
  • Agored Principles of Administration of Medication
  • Agored Fundamentals of Medication Administration
  • Attendance at relevant courses/study days
  • Successful completion of Agored Cymru accredited Unit on Medication Administration
Experience
  • Administration of medication via oral, topical, NG, PEG, JEG routes
  • Interpersonal and communication skills, including managing challenging situations
  • Recent clinical experience in a healthcare setting
  • Experience supporting development of others
Aptitude and abilities
  • Ability to work independently as an Assistant Practitioner
  • Competency in patient assessments within protocols
  • Effective relationship-building with patients, families, carers, and colleagues
  • Ability to supervise and assess trainees and learners
  • Fluent in Welsh
  • Ability to positively effect change

  • Participation in rotating shift patterns
