Supported Assistant Practitioner/Assistant Practitioner | 11041 | permanent contract | 37 hours per week | £30,023 to £32,046 per annum (F) or £33,119 to £34,811 per annum (G) | Grade F/G | Norwich

We are seeking an enthusiastic, compassionate and motivated Assistant Practitioner to join our dynamic Older People and Physical Disability (OP/PD) Team within Adult Social Care. This is an exciting opportunity to work alongside experienced Social Workers, Occupational Therapists and Assistant Practitioners within a forward-thinking service that is embracing innovative and strengths-based ways of working.

About The Role

This is an Assistant Practitioner role with a focus on Housing with Care (HWC) and Independent Living (IL).

You will work with individuals, carers and families to undertake assessments and support planning for adults wishing to move to a Housing with Care/Independent Living scheme. You will work with providers and landlords throughout the nomination and moving period to ensure the individuals have a smooth transition into HWC/IL, then complete their initial reviews. You will be a link worker for HWC/IL schemes within Norwich, ensuring a timely response to individuals’ changing needs and circumstances by completing reassessments and reviews to promote wellbeing and independence.

While the primary focus of the role is Housing with Care and Independent Living, there will be opportunities to work with adults in other settings. The role provides an excellent opportunity to build professional knowledge and experience whilst making a meaningful difference to people’s lives.

About The Team

You will be joining a highly experienced, welcoming and supportive Older People and Physical Disability Team, committed to delivering outstanding person-centred support. Our team supports adults who may be experiencing a wide range of complex circumstances, including physical disabilities, long-term health conditions, frailty, social isolation, self-neglect, safeguarding concerns and changing care and support needs.
