Job summary

Cardiff and Vale University Health Board is seeking an enthusiastic individual to support rehabilitation of stroke patients in their homes, under the guidance of qualified staff. The role involves MDT collaboration, data collection, and workload planning, with travel across the borough and surrounding areas.

You will work within the established interdisciplinary team based in Darlaston, delivering care in a community setting while progressing pathways for stroke recovery.

Qualifications

  • 5 GCSEs or equivalent.
  • Level 3 apprenticeship or Level 5 qualification.
  • Experience in an NHS health care environment working as part of a multi-disciplinary team.
  • Experience of working with stroke patients.
  • Experience of working in a community health care setting.
  • Experience of working in a rehabilitation setting, delivering Physiotherapy and/or Occupational Therapy care plans and/or SLT.
  • Able to assist registered staff in assessing a person's, delivery of therapy and nursing care, monitoring and reviewing the person's condition.
  • Able to work independently within limits of skill and knowledge and aware of when to seek advice.
  • Must be able to accurately follow instruction from a supervising health professional and to work as a team with all members of the department and Multidisciplinary Team
  • Strong communication skills.
  • Demonstrates the ability to travel frequently throughout the borough and surrounding locations during the working day.
  • Must be willing to work across the 7-day week as the service requires

  • Demonstrates the ability to work in MDT which include both clinical & non -clinical personnel, Knowledge of quality, audit and evidence-based practice

Job description

The successful candidate will assist in the rehabilitation of patients post stroke, in their own homes. With guidance from qualified staff members, they will facilitate client participation in their treatment / rehabilitation programmes for all disciplines. They will support the team with data collection and audits to continually improve the service. We are looking for an enthusiastic, motivated individual with a passion for stroke, in stroke to join our excellent established interdisciplinary team. The team consists of Physiotherapists, Occupational Therapists, Speech and Language Therapist, Specialist Nurses, clerical officer and Assistant Practitioners. The team is based in Darlaston, and works primarily in a community setting in the Walsall and surrounding areas., Valid driving license and access to a vehicle is essential.,
