A health and social care provider in Tavistock seeks an Assistant Practitioner committed to continence management. This role involves working independently within defined scope, supporting patients in managing their health, and collaborating with healthcare teams. Ideal candidates will possess a Level 5 qualifying education, effective communication skills, and the ability to work under pressure. Opportunities for growth and education are also provided.
We require a dedicated and compassionate Assistant Practitioner, who is inquisitive and enthusiastic about Continence Management and is able to work autonomously.
To work without direct supervision within the clearly defined boundaries of your competencies and carry out specific delegated clinical tasks and responsibilities, prioritising work in line with policies and procedures.
There are educational opportunities linked to role and the scope of Continence Management, this is a pivotal role requiring you to work collaboratively with the Continence Specialists Nurses, wider MDT and our patient community.
Promoting the highest standards of care in line with the latest guidance, research and information.
With appropriate information to help support of our Continence Specialist Nurses, there is much that patients can do to help themselves in the management and control of their condition.
This role will require you to support with educating our patients to self care, in the management and control of their condition, provide advice and support is now at the forefront of care.
Setting/AreaCovered: home setting, including residential, nursing homes and clinics.
This role may not be eligible for sponsorship under the Skilled Worker route.

The Assistant Practitioner will work as partof a team delivering therapeutic intervention, health and social care thatfocuses on the direct needs of the patients/service users. They will carry outspecific planned clinical tasks and responsibilities to facilitate, assessment,treatment, planning and evaluation of patients in a specific clinical team.
The post holder will work independently withintheir defined scope of practice under the delegation and guidance of aregistered professional. Applying specialist evidence-based knowledge ofprinciples and procedures within a specific field of healthcare.
Assistant Practitioners have undertaken abroad range of theoretical and practical learning regarding healthcare andwellbeing to Foundation Degree level.
Demonstrate and apply knowledge of the scope ofpractice of the Assistant Practitioner role within own clinicalsettings.
The post holder can deliver elements of health andsocial care and undertake clinical work in the domains that have previouslyonly been within the remit of registered professionals. The role at times may transcend professionalboundaries.
The Assistant Practitioner will have a level ofautonomy and accountability to make decisions around patient care and refer onto other professionals where required.

To undertake and to develop all nursing skills andcompetencies, updating as required for their role to enhance the skill mix ofthe nursing team ensuring that the ever-changing health needs of patients aremet.
To maintain accurate records, which are confidential,contemporaneous, legible, and all care given to be documented. These recordsmay be paper or IT based system and must be maintained as specified in the LWSWPolicies, the relevant professional body guidelines and Government directives.
To provide clinical supervision to other staff andstudents.
Promoting and supporting people in their care andtreatment to help them manage their own health and wellbeing, maximisingindependence and facilitating health promotion.
Depending on role you will be required to work within adefined geographical area undertaking specific clinical nursing duties topatients in a variety of settings such as their own homes, care homes, othercommunity locations or clinic settings.
To work within the scope of the NMC professional codeof practice. Moving across the organisation should the need arise, for example,in a pandemic situation.

Provide holistic evidence based / best practice care topatients in accordance with national and organisational approved policies /procedures and individual care plans.
To act as a clinical advisor on health promotionswithin their areas of expertise and act as a Link Nurse.
To undertake all required training to developcompetencies required for the role.

To be responsible in the absence of the Clinical NurseSpecialist / Caseload Holder to manage own workload and delegate nursing careto appropriately skilled staff and ensure that these are supported.
To assume responsibility as delegated by a senior member ofstaff.

To facilitate and participate in the development of teammembers, monitoring of sickness, line management, appraisals, mandatorytraining, competencies, clinical supervision and staff performance as directedby the Service Lead.
To participate in the recruitment and selection of newstaff.
To participate in coordinator/duty rota as required.
