Job description

A vacancy has arisen for an Assistant Practitioner in the cannock ESD/Community Stroke Team.

We are looking for an enthusiastic, motivated individual with an interest in stroke to join our excellent established interdisciplinary team. The team consists of Physiotherapists, Occupational Therapists, Speech and Language Therapist, Specialist Nurses, clerical officer and Assistant Practitioners. The team is based in Rugeley and works primarily in a community settings in the Cannock and surrounding areas.

Main duties of the job

The successful candidate assist in the rehabilitation of patients post stroke in their own homes. They will facilitate client participation in their treatment / rehabilitation programmes for all disciplines in a range of domiciliary settings. They will support the team with data collection and audits to continually improve the service. They will be responsible for organising, prioritising and planning their own workload to meet client and service priorities.

Walsall Manor houses the full range of district general hospital services under one roof. Person Specification
Qualifications
  • 5 GCSE's or equivalent
  • Level 3 apprenticeship or equivalent
Experience
  • Employment history working within a health care environment as part of a multi-disciplinary team
  • Experience of working with older people and people post stroke
  • Experience of working in a community health care setting
  • Experience of working in a rehabilitation setting, delivering Physiotherapy and/or Occupational Therapy and/or SLTcare plans
Skills
  • Able to work independently within limits of skill and knowledge and aware of when to seek advice
  • Demonstrable basic physical assessment skills
  • Able to assist registered staff in assessing a person's, delivery of therapy and nursing care, monitoring and reviewing the person's condition
  • Able to demonstrate empathy, motivate and encourage service users with impaired cognitive abilities or physical senses to participate in the treatment process
  • Ability to manage/prioritise workload autonomously
  • Possess a valid driving license and access to a vehicle
