An exciting opportunity has arisen for a Band 4 Assistant Practitioner to join the Newport Community Team on a permanent contract for 37.5 hrs (full time) over five days a week, including some weekends.
The Newport team provides clinical care and interventions for patients in their own home under the medical governance of Primary Care GPs.
The District Nursing/Community team in Newport works as a partner within a wider multi-disciplinary team which includes CRT Frailty Consultant, General Practitioners, Care at home, Physiotherapists, Occupational Therapists, Social Workers, Support and Wellbeing workers, and the Voluntary Sector.

An exciting opportunity has arisen for a suitably qualified Band 4 Assistant Practitioner to join the Newport community team in the provision of clinical care to individuals in their own home.
As Assistant Practitioner, you will work with the wider community MDT under the delegation of your Band 7, Band 6, and a team of Registered Nurses. You will utilise and develop a wide variety of skills to meet the emotional, physiological, and clinical needs of individuals.
The post holder will work collaboratively with and support members of the multidisciplinary team in the delivery, development, and management of health and appropriate referral for social care issues. The role is challenging but rewarding, and you will be fully encouraged and supported with your professional development.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
This post is permanent, part-time, over five days, and can include weekends.
