A major health service provider in Clevedon is seeking an Assistant Practitioner to support multi-disciplinary teams and deliver patient care. Applicants should have a Level 5 Foundation Degree and experience in healthcare settings. This role emphasizes professional development and patient management in a community setting. Full NHS benefits and a salary range of £27,485 to £30,162 pro rata are included.

This is an exciting opportunity for an Assistant Practitioner to work as part of our multi-disciplinary Integrated Neighbourhood Teams, mainly working within the therapy team.
By joining us you will have many opportunities to develop your clinical skills as well as physical assessment and clinical reasoning with the support of the wider team.
You will work as a member of the team to support the organisation and facilitation of clinical care in a holistic and proactive manner to patients in our service.

You will work within our service that is delivered from 8am-8pm, 7 days a week service.
You will be required to respond to referrals across urgent and planned pathways including hospital discharge facilitation.
You will work as part of the team to proactively monitor and manage patients on the caseload to ensure the best outcomes for them.
