Job description

An exciting opportunity has arisen for an Assistant Practitioner to join our Primary Care Home (PCH) to deliver high-quality care to the population of South Waveney, including Beccles, Bungay, Halesworth, and Southwold.

If you are caring, compassionate, and see nursing in the community as your career option, then this could be the job for you. We are seeking a passionate and motivated individual to join our team.

The PCH is a multidisciplinary team working closely with GPs and other community partners to provide an excellent standard of person-centered care to adults in their own homes, residential care, or clinic settings, supporting patients to self-manage whenever possible. The service uses SystmOne as a clinical IT system to support mobile working.

Previous experience of nursing in the community would be beneficial but is not essential. We will support you to transfer your existing knowledge and clinical skills as an Assistant Practitioner working in a different care environment.

The PCH is commissioned to provide a 24/7 service. The successful applicant will work 5 shifts of 7.5 hours between 08:30 and 16:30. The role includes weekend and bank holiday working on a rotational basis.

Main duties of the job

Within the PCH, you will work with the multidisciplinary team to provide planned and reactive community services. You will manage a delegated caseload of patients requiring nursing care, working autonomously within your scope of practice and under supervision of a registered nurse.

You will contribute to the assessment, provision, and monitoring of care, working in partnership with the team to deliver high standards of nursing care based on evidence.

Job responsibilities

Experience
  • Understanding of older people's needs, NHS/social care experience, multidisciplinary teamwork, and knowledge of Intermediate Care Services.
Qualifications
  • Foundation Degree in a health-related area or equivalent, evidence of personal development, and Care Certificate.
Skills and Knowledge
  • Good interpersonal, listening, and IT skills.
