Job Summary
An exciting opportunity has arisen for an Assistant Practitioner to join our Primary Care Home (PCH) to deliver high quality care to the population of South Waveney which includes Beccles, Bungay, Halesworth and Southwold.

The PCH is a multidisciplinary team working closely with GP's and other community partners to provide an excellent standard of person centered care to adults in their own homes, residential care or clinic settings, supporting patients to self-manage whenever possible. The service uses SystmOne as a clinical IT system to support mobile working.
Previous experience of nursing in the community would be beneficial but is not an essential requirement. We will support you to transfer your existing knowledge and clinical skills as an Assistant Practitioner working in a different care environment.
The PCH is commissioned to provide a 24/7 service. The successful applicant will be required to work 5 X 7.5 hours shifts between the hours of 08:30 and 16:30. The post includes weekend and bank holiday working on a rotational basis.

Main duties of the job
Within the PCH you will work in partnership with other members of the multidisciplinary team to provide planned and reactive community services. The post holder will provide clinical case management for a delegated caseload of patients living in the community who require nursing care.
The post holder is required to work autonomously at a level appropriate to clinical competence and within scope of professional practice.
The post holder will work under the supervision and working in partnership with a registered nurse, provide and deliver a high standard of nursing care contributing to the on-going assessment, provision and monitoring of care based on evidence and working in partnership with the multidisciplinary team.

- Take, record and report blood pressure, respiration, pulse rate and oxygen saturation levels as necessary. Be able to recognise the deteriorating condition.
- Undertake and record MUST and Waterlow assessments and take appropriate action.
- Refer to social care services as required.
- Discharge patients from service.
- Arrange care packages in accordance with protocol.
- Complete Generic Assessment on System 1.
- Complete and allocate care plans and manual handling plans for patients.
- Prescribe exercise and mobility programmes within the scope of practice.
- Assess level of support required with medication (Administer, Assist, Prompt, None).
- Participate in the promotion of education and training to clients and carers.
- Check compliance with medication Prompt, Assist and Administer medication in accordance with care plan.
- Initiate, delegate and progress/adapt treatment plans for non-complex patients.
- Assist the senior staff with admissions to reablement beds and complex situations/rapid response in the community.
- Assess, provide and fit small aids and equipment within the protocols.
- Order therapeutic and rehabilitative equipment.
- Receive and accurately document highly complex information.
- Take responsibility for the safe use of clinical equipment.
- Contribute to the effective use of resources.
- Report any concerns/changes in a patient's condition to a registered health care professional without delay.
