Qualifications

Job description

Management of clients and responsibility for their ongoing care and support with a focus on person-centred care.

  1. Carry out client reviews, quality assurance visits, end of service reviews, and take appropriate action.
  2. Conduct client and Care Professional introductions.
  3. Create, update, and audit client care plans; assist with digital care planning.
  4. Perform regular audits on client activity logs and MAR charts; update care plans accordingly.
  5. Identify training needs based on audit activities and follow up.
  6. Maintain accurate records of clients and Care Professionals using Home Instead software and People Planner.
  7. Build and maintain positive relationships with clients, their families, and other professionals involved in their care.
  8. Participate in on-call duties as required.
  9. Ensure compliance with Home Instead's Equality, Diversity, and Equal Opportunities Policy in employment and service delivery.
  10. Perform any other duties necessary for the successful operation of the business.
  11. Perform any additional duties as directed by your line manager.

Qualifications and Experience:

  1. Level 3 NVQ in Health and Social Care, equivalent, or working towards.
  2. Minimum of 12 months experience in the care sector delivering personal care services.
  3. Passion for delivering high-quality care and supporting clients to live independently and happily at home.
  4. Ability to nurture CARE-Professionals to reach their full potential.
  5. Knowledge of legislation and regulations specific to Health and Social Care.
  6. Good communication skills and ability to build rapport quickly.
  7. Understanding of confidentiality and current legislation.
  8. Proficiency in IT systems, including Microsoft Office or Google Suite, and virtual communication platforms; willingness to learn new technologies.
  9. Full driving license and means of transport if required within the territory.
  10. Organized and flexible to meet business needs.
