Job description

Derbyshire Dales Primary Care Network Ltd (DDPCN Ltd) is seeking a proactive and compassionate Assistant Care Coordinator to join our team. This role will support the delivery of person-centred care by working alongside our existing Care Coordinators to identify and address the non-clinical needs of patients with complex health and social circumstances.

The post-holder will provide operational and administrative support to the Care Coordination function, including the coordination of our Home Visiting Team and the Home from Hospital / Transfer & Settle Service, ensuring smooth, timely, and joined-up care for patients across the PCN. The Assistant Care Coordinator will also provide cover during periods of absence within the Care Coordinator team, ensuring consistency of service delivery across practices.

Main duties of the job
Administrative and Team Support
  • Provide general administrative support to the Care Coordination team including appointment scheduling, call handling, correspondence, and documentation.
  • Attend and contribute to MDT (multi-disciplinary team) meetings, taking notes and following up on assigned actions.
  • Provide cover and continuity during periods of Care Coordinator leave or absence.
  • Contribute to internal audits, evaluations, and service reviews to support continuous improvement.
General Care Coordination Support
  • Assist Care Coordinators in identifying and supporting patients who would benefit from personalised care planning, including those with long-term conditions, frailty, or frequent hospital admissions.
  • Support patients in understanding and navigating local health and social care services, promoting self-management and independence.
  • Maintain up-to-date and accurate records in clinical systems, ensuring compliance with GDPR and local governance.
Home Visiting Team Coordination
  • Manage daily referrals and scheduling for the PCN Home Visiting Team, ensuring efficient allocation of visits.
  • Liaise with GP practices, community services, and visiting clinicians to prioritise and coordinate patient visits.
  • Track visit outcomes and ensure follow-up actions are communicated and documented.
Home from Hospital / Transfer & Settle Service
  • Coordinate referrals from acute and community hospitals into the Home from Hospital or Transfer & Settle pathway.

DDPCN supports the 7 GP Practices situated in the beautiful Derbyshire Dales. The main office is at Scholes Mill, Tansley near Matlock, which has plenty of free on-site parking.

As outlined previously (above). This is a part-time role (0.5 FTE) with flexible working hours. Some travel across the Derbyshire Dales area may be required. The role may occasionally involve attending meetings at partner sites. Full training and supervision will be provided. This post may be combined with a 0.5 FTE Finance and Administration Assistant post for a full-time opportunity.
