Job description

Assistant Care Co-ordinator – Dudley Disability Service

Hours: 37 per week, Permanent – full-time

Salary: Grade 6 (£27,711 - £30,060)

About Us

The Dudley Disability Service is seeking to recruit a permanent Assistant Care Coordinator. This is an exciting and rewarding opportunity to work with a developing and motivated team dedicated to supporting people, their families, and carers to explore, plan, and access appropriate housing, support, social, leisure, learning, and employment opportunities.

We are looking for a creative and enthusiastic individual who can ensure a strengths-based approach in practice, promoting wellbeing and independence by supporting individuals to access resources, make informed choices, and build new relationships. This will enable them to live in their own homes and participate as active citizens of Dudley.

Key Responsibilities
  1. Support the operations of the service, including duty management.
  2. Respond to a wide range of contacts and agencies.
  3. Contribute to the development of Multi-Disciplinary work.
  4. Support individuals to maximize independence, improve health and wellbeing in a coordinated manner.
  5. Manage tasks with oversight, identifying adults at risk of abuse or neglect.
Candidate Requirements
  • Experience in adult social care.
  • Commitment to empowerment and strengths-based approaches.
  • Dedication to diversity and a non-judgemental attitude.
  • Knowledge of adult social care, mental health, and mental capacity legislation is beneficial.
  • Experience working with adults with care and support needs is desirable.
