Rochdale Adult Care offers an opportunity for Assessment and Support Planners to join Adult Care Services within the Adult Care Localities and the Hospital Discharge Hub. This role provides a chance to work within a vibrant and innovative authority.

The successful candidate will be dynamic, fast-paced, and possess excellent written and communication skills, with a willingness to learn. The role involves working with Adults, their Carers, and hospital professionals, adhering to timescales and deadlines to produce high-quality work. In return, you will have opportunities to develop your skills within a supportive environment.

Applicants should have some knowledge or experience of working within an Adult Care setting, including strength-based approaches to assessments and support planning, promoting independence and well-being, in accordance with the Care Act 2014.

This position is ideal for individuals interested in gaining practical experience working with adults in a supportive setting. Training and development opportunities are available.

The teams within the Integrated Neighbourhood Teams operate across Heywood, Middleton & Hospital Hub, and Pennine Localities, covering a wide range of work with Adults over 18.

Applicants must be able to travel around Rochdale and be available for a seven-day working rota.

Appointment is subject to an enhanced DBS check, including a barred list check for adult workforce.
