Job Description:

Rochdale Adult Care is pleased to offer an opportunity for an Assessment and Support Planner to join our vibrant and creative authority. The role will be based in adult care’s Front door Prevention team. This service supports people at the earliest opportunity and maximizes the effectiveness of early intervention and prevention, aiming to prevent or delay individuals from developing longer-term care and support needs. This enables individuals to be healthy for as long as possible, self-managing long-term conditions.

There is 1 permanent post available. The post holder will be expected to have experience in assessment and support planning, including offering advice and guidance to service users and carers, within the objectives of the Care Act 2014.

We expect our Assessment and Support Planners to use strength-based approaches to promote the independence and well-being of service users and carers, utilising natural support and the voluntary sector where necessary.

The ability to work within timescales and deadlines is necessary, as is the ability to write care records and reports clearly and concisely.

In return, you will have the opportunity to further develop and progress your skills through undertaking a variety of appropriate training and development courses.
